Renal stones are solid particles in the urinary systemThey are one of the most painful of the urologic disordersMen tend to be affected times than women. It occurs more frequently in whites than in blacks.It shows familial predisposition. It is common in people those in tropical countries. Studies have shown that 4% of the total population has stones in the urinary tract.

TYPES OF STONE

Calcium oxalate

Hard, single, mulberry. Irregular in shape with sharp projection and radio opaque.

Uric acid 

Hard, smooth, small, often multiple, yellow or brownish red color and radiolucent. Occur in acid urine.

Cystine stone

Multiple, radio opaque and amber or yellow. They occur in acid urine. Appear in early childhood with congenital error of metabolism.

Phosphate stone

Smooth, round, dirty white to yellow in color and radiolucent.

Mixed stone

Composed with Magnesium, Ammonium and Phosphate.

Xanthine stone

Smooth, round and brick red in color.


ETIOLOGY

Inadequate urinary drainage.

Dehydration.

Decreased urinary citrate.

Deficiency of vitamin A.

Diet with high acids and oxalates.

Renal infection.

Prolonged immobilization.

Symptoms

Acute pain at lower part of back or abdomen.

It radiates to perineum or buttocks or thigh.

Pain is increased on movement.

Acute 
symptoms may accompanied with nausea, vomiting, dizziness and abdominal 
discomfort. Polyuria, oliguria, or haematuria may be occurred in some occasion.

General measures

Increase fluid intake 6 to 10 glasses per day in summer. Take water 60 ml/kg body weight.

Hot fomentation to relive pain.

Hot fomentation to promote urine flow.      

Hot Epsom salt compress.             


Calcium stone

Maintain high urine volume.

Avoid sun exposure and source of vitamin D in summer.

Prefer vitamin B 6 rich food for oxalate stone.

Vitamin B6 rich items: Banana, Grapes, Fish, Chicken, Germinated grains, Nuts, Walnut, Flax seeds, 

Sun flower seeds.

Avoid Phosphorous rich soft drinks.

Avoid high dose of vitamin C supplements.

Correct endogenous production of oxalates from vitamin C and other source (>90%).

Keep Calcium to: Phosphorus ratio in diet 2:3.

Prefer good calcium: phosphorus diet like Banana, Oats, Barley, and Rice.

Keep carbohydrate to fat ratio to 4:1.

Restrict animal proteins rich in organic acids.

Take fiber rich carbohydrate.

Restrict 
calcium rich items: Milk items, Liver, Prawn, Carrot, Bengal gram, Spinach. Tomato, Nuts. Sesame seeds, Onions big, Coconut, Cabbage, Chocolate, Egg, Calcium rich antacids.

Promote citrate concentration in urine. 

Take 
citrate rich items like Citrus fruits, Orange, Lemon, Pumpkin, Banana, and Sardine.


Phosphate stone

Restrict phosphate rich diet.

Correct infection properly.

Avoid high alkaline diet.

Avoid oxalate rich items: Chocolate, Cocoa, Tea, Spinach, Rhubarb, Beet tops.


Uric acid stone

Take low purine diet like Red meat, Sardines, Tuna, Alcoholic drinks, Chocolates and Tea.

Avoid phosphorous rich soft drinks.

Restrict acid diet items like Bakery items, Fried food, and Meat.


Cystine stone

Avoid sulphur containing proteins like Egg, Meat and Fish.

Take magnesium rich food like Green vegetables and Fruits.


Other measures

Avoid urinary stagnation and urinary salt concentration.

Avoid excess of refined carbohydrate and sugar.

Restrict common salt less than 3gm per day.

Prefer potassium rich item like Water melon, Pumpkin, Tender coconut, Grapes, and Boerhavia diffusa.

Restrict 
medicine like Allopurinol, Aspirin, Sulfa groups, Ephedrine, Ciprofloxacin, Carbonic anhydrase inhibitors, Magnesium trisilicate, Acetazolamide and Furosemide.  

Avoid Pepper, Shellfish, Bengal gram, Laxatives and Sugar in summer.

Prefer rain water and distilled water.

Prefer Carbo vegetabilis in renal insufficiency.
